Breckland District Council's social prescribing and health-and-wellbeing programme, delivered in partnership with Community Action Norfolk.
Breckland Councils social prescribing service places Community Connectors alongside local Primary Care Networks and GP practices across Thetford, Dereham, Swaffham, Watton, Attleborough and surrounding villages. Connectors meet residents who feel lonely or isolated, have long-term conditions, mental wellbeing needs, money or housing worries, or are unpaid carers. They listen, then link people to community activities, voluntary groups and wider support. Residents can be referred by their GP or another health professional, or self-refer online.

Who they help: Adults in Breckland facing isolation, long-term conditions, mental health or financial issues
Eligibility: Adults living in the Breckland district who would benefit from non-clinical support for their health and wellbeing.

Not suitable for: Service covers Breckland district only. Residents elsewhere should contact their local social prescribing service. Children and young people should be referred to Just One Norfolk or schools-based support.
