Free social-prescribing service for adults registered with an Ipswich GP, linking people to community activities, financial help and emotional support.
Connect for Health is the social prescribing service delivered by Citizens Advice Ipswich for adults aged 18 and over who are registered with a GP surgery in Ipswich East or Ipswich West. Link Workers see people in their GP surgery and tackle the wider issues that affect health - loneliness, social isolation, financial worries, housing problems and getting back into work. They connect people with voluntary and community sector activities, welfare benefits advice and specialist Citizens Advice services. Referrals come from Ipswich GP practices only.

Not suitable for: Referrals are only accepted for patients registered with an Ipswich GP. People registered with GPs in Woodbridge, Felixstowe or the coastal area should ask for Connect for Health East Suffolk via Shaw Trust instead.
