Shaw Trust-delivered social prescribing for adults registered with GPs in Woodbridge, Felixstowe, Kesgrave, Martlesham, Wickham Market and surrounding coastal areas.
Connect for Health East Suffolk is the free Shaw Trust social prescribing service for adults aged 18+ registered with a Suffolk GP. Community Connectors meet you, agree a personalised action plan and link you to local groups, benefits help and wellbeing services, usually over about three months. You do not need a GP referral; self-referrals are welcome. Phone 01394 332265 or email c4h.suffolk@nhs.net. Specialist routes cover Hospital to Home and respiratory conditions.

Who they help: Adults living with loneliness, low mood, long-term conditions, housing, money or employment worries
Eligibility: Adults aged 18+ registered with a GP in East Suffolk (Woodbridge, Felixstowe, Kesgrave, Martlesham, Wickham Market and coastal villages).
Area covered: East Suffolk
Not suitable for: This service only takes referrals for adults registered with an East Suffolk GP. People registered with an Ipswich GP should ask for Citizens Advice Ipswich Connect for Health. Patients under 18 should be referred to youth services such as 4YP.
