Person-centred social prescribing for adults registered with a City & Hackney GP — link workers connect residents to local activities, advice and support.
City and Hackney Social Prescribing is delivered by Family Action across eight Primary Care Networks: Clissold, Hackney Downs, Hackney Marshes, Woodberry Down and Wetlands, Spring Hill, London Fields, Shoreditch and The City, and Well Street Common. Adults aged 18 and over registered with any local GP can self-refer or be referred. After an assessment, a social prescriber helps people connect with arts, exercise, learning, volunteering, befriending and specialist services that match their needs.

Who it's for
Who they help: Adults experiencing social isolation, financial difficulty, low-level mental health needs or long-term conditions.
Eligibility: Adults 18+ registered with a GP in City or Hackney; referrals from GP practices.
Area covered: Hackney
Not suitable for: This service is only for adults aged 18 or over who are registered with a GP in the City of London or Hackney. People outside this area should contact their local social prescribing team.
