Foundry Healthcare Lewes runs an in-house social prescribing team with link workers split by age group: young people 8-20 (Claude Lovett), adults 21-49 (Donna Tookey) and over-50s (Alison Locke and Maria Graves). Michelle Whatford supports patients living with dementia or frailty. Link workers connect people with community resources for issues like loneliness, stress, money worries and long-term condition self-management.
