Pioneering Frome-based social prescribing and health coaching team — the original Frome Model.
Health Connections Mendip is the social prescribing and health coaching service for 11 GP practices in the Mendip area, including Glastonbury Surgery, Wells Health Centre and Frome Medical Practice. Health Connectors offer one-to-one appointments, group health and wellbeing programmes, Talking Cafes for peer support, an online directory of local resources and community training.
