Social prescribing service for adults across Wakefield district, connecting people to community support for non-medical needs.
Live Well Wakefield is a social wellbeing service delivered by Nova Wakefield District and South West Yorkshire Partnership NHS Foundation Trust. It supports adults 18 plus living in Wakefield or registered with a Wakefield GP, helping with non-medical needs such as loneliness, housing worries, finances and long-term conditions. Link workers create personalised action plans and connect people to community groups, peer support and self-management programmes to improve health, independence and wellbeing.
