Free rapid home assessment and short-term crisis support for Basildon and Brentwood residents with dementia in crisis.
The NELFT Dementia Crisis Support Team works with adults over 18 who have a diagnosed or suspected dementia in Thurrock, Basildon and Brentwood. The team's aim is to reduce the need to access A&E and avoid hospital admission during a crisis period. They engage with the patient and carer in their own home or usual place of residence, providing rapid assessment, intervention, treatment and short-term follow-up for a maximum of six weeks. Referrals from GPs, community nurses, social workers and other health and social care professionals.
