NHS community diabetes specialist nursing, education and self-management support for Newham adults with Type 1 and Type 2 diabetes.
The Newham Community Adult Diabetes Service is delivered by East London NHS Foundation Trust and includes diabetes specialist nursing, the structured education programme for people with Type 2 diabetes, and pre-diabetes education. The team supports adults to understand their condition, learn how to manage it and reduce complications, working closely with primary care and hospital teams.

Who they help: Adults with diabetes or pre-diabetes needing education and support.
Eligibility: Adults registered with a Newham GP with pre-diabetes, Type 1 or Type 2 diabetes.

Not suitable for: You need a referral from your GP. If you live outside Newham, please ask for a referral to your local community diabetes service.
