Free NHS 12-week online weight management programme for adults living with obesity who also have diabetes, high blood pressure or both — delivered via smartphone or web
The NHS Digital Weight Management Programme is a free 12-week online programme for adults living with obesity who also have a diagnosis of diabetes, high blood pressure or both. Delivered entirely via smartphone or computer by established providers (including Second Nature, Oviva and Slimming World), each plan offers recipes and nutrition advice, wellbeing support and tips to increase activity levels. Access is by referral from a GP practice or community pharmacy, or by online self-referral at public.wmp.nhs.uk — after referral you receive a text message within 2 working days inviting you to choose a provider and start the programme.

Who it's for
Who they help: Adults 18 and over
Eligibility: Adults 18+ with a BMI of 30+ (27.5+ for people from Black, Asian and some other ethnic backgrounds) AND a diagnosis of diabetes, high blood pressure or both. Referral via GP practice or community pharmacy, or online self-referral.
Not suitable for: Not available without a qualifying diagnosis (diabetes and/or high blood pressure); not suitable during pregnancy.
