Multi-agency Help Hub run by South Norfolk and Broadland Councils offering support, social prescribing and Community Connectors.
The South Norfolk and Broadland Help Hub brings together district council officers and partner agencies to offer practical help to residents on issues affecting health and wellbeing. The team includes Community Connectors who work as a social prescribing link service, helping residents access local community activities, mental health support, financial advice, housing support and adaptations. The Help Hub also incubated the District Direct hospital discharge model.
