Borough-wide social prescribing run by Tower Hamlets GP Care Group — each GP practice has an allocated social prescriber for adults aged 18 and over.
The Tower Hamlets GP Care Group has run the boroughs Social Prescribing service since 2016. Any patient registered with a Tower Hamlets GP, aged 18+, with a non-clinical need such as social isolation, low mood, housing, money or employment worries can self-refer or be referred by their GP, nurse or any local primary care professional. Each practice has a named link worker, usually based on site with access to patient records, who arranges signposting, follow-up appointments and links to local voluntary sector activities. Over 11,800 people were referred in 2024/25.

Not suitable for: This service is only for adults aged 18 or over registered with a Tower Hamlets GP. Children, young people and people outside the borough should contact their own GP practice.
