Council-led social prescribing scheme with link workers placed in every Waltham Forest GP surgery, helping residents connect with community support for non-medical needs.
Waltham Forest Councils Public Health team set up social prescribing in 2016. Today the service has six Social Prescribers and two Senior Social Prescribers/Project Coordinators across the seven Primary Care Networks, plus a council link worker. Patients aged 18+ are referred by their GP or another health professional for help with cost of living, benefits and debt, food security, digital inclusion, energy and heating, physical activity, healthy eating, bereavement and long-term conditions. Not suitable for: You must be registered with a Waltham Forest GP to access this service. Residents registered with GPs outside the borough should ask about local equivalents. Referral is usually via your GP surgery.
