Free Lincolnshire County Council service helping adults stay safe and independent at home, including home from hospital support.
Wellbeing Lincs is a free countywide service funded by Lincolnshire County Council, helping adults aged 18+ live more confidently and independently at home. The Home Safe element provides short-term support transporting people home from hospital and helping them settle in. The wider service helps with managing long-term health conditions, reducing falls and unplanned hospital visits, equipment and minor adaptations, and links to community support to prevent crisis.
